Adult Protective Services Program

What information do I need?

The screening social worker will request the following information:

• Your name (or you may request anonymity if you are not mandated)
• The elder or dependent adult’s name, address, telephone, age, and current location
• A description of the circumstances which lead you to believe abuse/neglect is occurring
• Any other information you may have regarding the name of the individual believed to be responsible for the abuse/neglect
• Date, time and place of incident
• Any statement made by the victim regarding abuse/neglect

Results of investigations are kept confidential
