Homelessness has a significant impact on the health and quality of life of individuals living without homes and the entire community. In November 2020, the Santa Cruz County Housing for Health (H4H) Division was established to unite a coalition of partners and resources with the goal of preventing and ending homelessness in our community.
 
The Housing for Health Partnership (H4HP) serves as the federally designated Continuum of Care (CoC) for Santa Cruz County, and the H4H Division provides staffing support for the H4HP. The partnership includes individuals working for the County, cities within the county's jurisdiction, local non-profit organizations, housing and service providers, individuals with lived experience of homelessness, and other community members. All members of the partnership recognize the critical role that collaboration will play in addressing homelessness.
 
The H4HP is committed to reducing homelessness, assisting those at risk of losing housing, and advocating to close the housing affordability gap in Santa Cruz County.
