Request for Proposals and Related Solicitation Documents

The County of Santa Cruz, Human Services Department administers several local, state, and federally funded programs.  These programs frequently require services from qualified contractors, vendors, and community-based organizations. Listed below are open requests and recently closed requests for proposals and/or requests for qualifications released previously by the Human Services Department. OPEN REQUESTS FOR PROPOSALS:

Request for Qualifications for Housing and Homelessness Supportive Services

On April 30, 2024, The County of Santa Cruz Board of Supervisors allocated $1.5 million ($500,000 per year for three years) in Collective of Results and Evidence-based (CORE) funds from the FY 2025-28 funding cycle to support housing and homelessness prevention services. These funds will be managed by the Housing for Health (H4H) division within the County Human Services Department (HSD).

As a result, H4H is seeking proposals from Qualified H4H Vendor Pool organizations to help people keep stable, safe, healthy housing and living situations in the southern region of Santa Cruz County. Find a copy of the full RFP here.

Applicants MUST be a pre-qualified H4H Vendor Pool supportive services vendor to qualify for this opportunity. Organizations interested in applying for this opportunity that are not already a qualified vendor must submit a Vendor Pool application along with their application for this specific opportunity. H4H is contracting for the following homelessness prevention services:

  • Data-informed, targeted outreach and engagement
  • Housing stabilization services
  • Flexible financial assistance
  • Housing-related legal services

A single, lead vendor will be selected based on their project proposal, organizational staffing capacity, experience, community connections and ability to provide the services as outlined in the Request for Proposals (RFP). The successful respondent will demonstrate special skills and experience by providing the following:

  1. Obtaining approval as a Housing for Health Vendor Pool supportive services provider
  2. Completing a narrative application questionnaire
  3. Providing a detailed program budget
  4. Committing to participate the Santa Cruz County Continuum of Care (CoC) as an “Organizational Member” and using the Homeless Management Information System (HMIS) for program data collection and reporting.

H4H intends to enter into a formal agreement by July 1, 2025, with a three-year term and an annual budget of $500,000.

RECENTLY CLOSED REQUESTS FOR PROPOSALS:

COMPREHENSIVE PREVENTION PLAN (CPP) LIVED EXPERIENCE PILOT PROJECT – REQUEST FOR PROPOSALS #2024HSD03
The County of Santa Cruz Human Services Department is seeking proposals from qualified organizations, to uplift and amplify the voices of people with lived experience of the child welfare system, by providing targeted resources for their participation in all aspects of planning, implementation, and evaluation of the County’s Comprehensive Prevention Plan (CPP).
 
Human Services and Juvenile Probation are the County lead agencies for the CPP, and have engaged a cross-sector Child, Youth, and Family Well-being Cabinet to develop and implement a plan for and with the community. The CPP aligns with the California Department of Social Services (CDSS) Family First Prevention Program and describes the local approach to building systems for prevention so that more children and youth can stay safely in their homes and in their community. CORE INVESTMENTS FISCAL YEARS 2025-2028 – REQUEST FOR PROPOSALS
The County of Santa Cruz (County) and City of Santa Cruz invites proposals for the Collective of Results and Evidence-based (CORE) Investments funding opportunity. Submit proposals by 5:00 p.m. Pacific Time (PT) on August 2, 2024. All proposals must be submitted via an online proposal system. This opportunity aims to uncover and address the root causes of inequities to advance the CORE Conditions of Health and Well-Being for all residents of the County. Proposals will be accepted from non-profit 501(c)(3) entities, federally recognized Tribal entities, and tax-exempt education entities. Nonprofit 501(c)(3) acting as fiscal sponsors or a lead fiscal agency are eligible to apply for the program(s) for which they provide fiscal oversight. The County of Santa Cruz Human Services Department invites proposals to identify qualified Co-Applicants for the California Department of Housing and Community Development (HCD)  Project Homekey Round 3 Grant Program to fund supportive housing projects.  Organizations must have the ability to meet HCD Submission requirements as outlined in Attachment A Homekey Guidelines - Notice of Funding.
