Whether you are calling to make a referral for IHSS on your own behalf or on behalf of someone else, please have as much of the following information as possible:
It is important to let the staff person know if you or the person on whose behalf you are calling is terminally ill, if there is a mental impairment, a caregiver is involved, there are issues of illiteracy, and/or what other agencies may be providing services.
While not necessary, it is of benefit to provide the name, address and phone number of an emergency contact, should we not be able to reach you or the applicant, if you’ve called on behalf of someone else.
